How do I sign Up?

Fill out the form below and print a copy of this page after you have submitted the form. Please include one copy of each child’s birth certificate (these are only used for verification and are promptly shredded afterwards).

What can I expect?

After you submit the form, please print this page and mail along with a copy of each child’s birth certificate. The mailing address will be shown after submitting your application. Please allow 2-4 weeks for delivery of your welcome package.

    To qualify for the program, you must submit a printed copy of this page along with a valid copy of each child's birth certificate. Only proud parents of triplet births or more qualify. Birth certificates must be dated within the past 1 year or will be considered invalid. US Residents only. We reserve the right to reject any application for any reason. Void where prohibited.
